Pediatric Case Manager - MSW/LCSW
HealthCare Support is actively seeking a Pediatric Case Manager - MSW/LCSW to fill an opening with a Fortune top 25 Managed Care Company in Sunrise, FL. This is a Hybrid role Working Remote and in office on Thursdays in Sunrise Florida 33323. Additionally, this position may require some field work, such as home/skilled nursing facility visits.

Company DescriptionHealthCare Support is actively seeking a Pediatric Case Manager - MSW/LCSW to fill an opening with a Fortune top 25 Managed Care Company in Sunrise, FL. This is a Hybrid role Working Remote and in office on Thursdays in Sunrise Florida 33323. Additionally, this position may require some field work, such as home/skilled nursing facility visits.
Daily Responsibilities for Pediatric Case Manager - MSW/LCSW:
- Mon- Fri: 830a-5pm
- Identify special needs members through the completion of health screens and other resources
- Work with community outreach/member advocates to coordinate member care
- Educate providers and community resources on program components and available support services
- Educate members with special needs to foster compliance with program and positively impact outcomes helping complete the monthly and quarterly reports that come with that transition.
- Home visits may be required for discharge assessments depending on the situation.
Required Qualifications for Pediatric Case Manager - MSW/LCSW:
- Masters of Social Work (MSW) Required
- 2+ years of experience as a social worker managing and integrating the social/community needs of members
- Experience with Medicaid Program and CMS Guidelines.
- Knowledge of Microsoft Office and internet software
- Ability to work telephonically
- Strong documenting experience in multiple systems
Preferred Qualifications for Pediatric Case Manager - MSW/LCSW:
- LCSW Highly Preferred
- Bilingual in English/Spanish
- Behavioral health experience preferred
- Managed Care/Health Plan experience HIGHLY Preferred
