Our collaborative model involves both MDs and PTs, eliminating the need for you to hunt for a physician when you need a new prescription or have concerns. Our typical schedule entails seeing 10-15 patients per day. We prioritize early injury detection and intervention, resulting in faster recovery and a quicker return to work for our patients. This is physical therapy as it should be.

Work Schedule: Monday to Friday, 7:00am to 6:00pm, on a PRN basis
Responsibilities:
- Evaluate and assess patients' physical therapy needs, formulate specific treatment plans and goals, and implement these plans with physician approval.
- Completion of all required documentation in an accurate and timely manner:
- Initial evaluations
- Daily treatment notes
- Staffing reports
- Discharge summaries
- Timely reassessment
- Educational activities for patient
- Billing-related documentation
- Insurance justification letters for equipment purchase and rentals
- Other patient care-related documents as needed
- Provide patient educational services, both verbally and in writing, documenting all educational activities and patient responses in charts.
- Supervise the administration of physical therapy services by the Physical Therapy Assistant as needed.
- Participate in treatment plan reviews, patient case review conferences, patient assessments and reassessments, and staffing with physicians.
- Adhere to the company's established policies and procedures, including strict patient confidentiality.
- Assist the Medical Director as directed in the following, but not limited to: Utilization Review & Quality Assurance
- Perform other duties as assigned.
Requirements:
- Bachelor’s, Master's or Doctorate in Physical Therapy from an accredited institution.
- Must have a license (in good standing) or diploma to practice physical therapy from the State of Arizona.
- Current CPR Certification.
