Overview
Provide nursing/health care services for residents within the Adult day program.
Responsibilities
- Initial and continuing observation and assessment of each participant in the program by the nurse in the multi disciplinary team and development of a plan of care. The nurse shall assess each participant to determine needs for personal care and assistance in activities of daily living such as bathing, grooming, transferring, and feeding; include these needs in the plan of care and supervise the provision of these services, and specify long and short term goals for each participant.
- Administration of medications or monitoring of self-administration of medication by participants. The nurse shall monitor, administer and record prescribed medications.
- Supervision of personal care given by program aides. The nurse shall supervise the activities of the program aides and other health care staff when they provide personal care to the participants.
- Maintenance of a participant’s health record which includes: Monthly progress notes from each member of the multi disciplinary team and outside consultant. Records of medications and treatments administered. Monthly progress nursing note, or more frequently if necessary, indicating participant’s progress toward achieving health goals.
- Provision of emergency care, including arrangements for transportation. The nurse shall assess any urgent patient care situations to determine the appropriate course of action, provide immediate emergency care and arrange for transport to obtain medical
- Notification of the physician of any marked change in the participant's condition. Coordination of the treatment plan with the participant's personal care physician. The nurse shall be the liaison between the Center and the participants' personal Other responsibilities not limited to the above description.

Qualifications
- Current Massachusetts nursing license. Only Registered Nurses are hired; First Aid/CPR Certification.

Experience:
- Nursing: 1 year (Preferred)
License/Certification:
- CPR Certification (required)
- RN (Required)
