LICENSED PRACTICAL NURSE

SUMMARY:
Under supervision of a registered nurse, provides quality care to patients/clients and performs technical skilled care in patient/client care settings as assigned in compliance with nursing standards, federal, state, local government regulations and company clinical policies.
QUALIFICATIONS:
To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. The requirements listed below are representative of the knowledge, skill, and/or ability required.
· Provides direct and individualized nursing care to assigned patients based on nursing standards and under the supervision of the Director of Nursing and/or a Registered Nurse designee.
· Ensures quality and safe delivery of health care services to patients/clients.
· Implements plan of care formulated by physician and Registered Nurse.
· Provides accurate and timely documentation consistent with plan of care.
· Assesses and provides patient/client and family caregiver education and information pertinent to diagnosis and plan of care under the supervision of the Registered Nurse.
· Maintains safe and healthy working environment and uses safe work methods and procedures in accordance with company clinical standards.
· Assists patient/client in learning appropriate self-care activities
· Uses equipment and supplies effectively and efficiently.
· Documents and reports activity as required.

EDUCATION AND/OR EXPERIENCE
· Licensed as a licensed practical nurse in the state of Maryland
· Graduate of an accredited practical nurse program
· Minimum of one year of nursing experience.
COMPETENCY/SKILL REQUIREMENTS:
· Must have excellent written and verbal communication skills
· Able to utilize the telephone for internal and external communications
· Good knowledge of nursing assessment and observation skills
· Must be competent in the provision of nursing care
· Current CPR certification
PHYSICAL DEMANDS:
· The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job.
· The work requires moderately heavy physical exertion on a regular and recurring basis such as: assisting patient in transfer activities (wheelchair, to bed, to tub, to commode) and providing substantial support to patients/clients in ambulation.
· Employee is required to stand; walk; stoop and must occasionally lift and/or move over 50 – 100lbs.
