Overview of the Position: Under the direction of the Director of Patient Care Services, the Home Health Physical Therapist develops and implements a plan for providing physical rehabilitation and home therapeutic treatment, as well as education of patients, family members, and significant others. Service areas are San Luis Obispo and Northern Santa Barbara Counties.

ESSENTIAL FUNCTIONS OF THE JOB SUMMARY
- Develops the physical therapy treatment plan and provides skilled physical therapy services required by the plan.
- Evaluates the patient's level of function by applying diagnostic procedures and develops/revises the physician's plan of treatment based on best practice principles and in accordance with all applicable licensure definitions and commonly accepted professional standards of practice.
- Performs direct physical therapy treatments based upon assessments and treatment plan.
- Educates and trains patients/family on patient's total therapy program including additional exercises and equipment needed to aid recovery.
- Keeps accurate visit records with mileage and submits time sheets with attached documentation on a daily basis.
- Evaluates effectiveness of treatment plan and modifies plan based on changing needs of the patient.
- Ensures coordination among all relevant parties to maintain continuity of care and communicates effectively with team members, families, patients, physicians, referral sources and community agencies to keep all informed of patient's progress and/or condition.
- Participates in in‐services and patient care conferences to increase job knowledge.
- Supervises the clinical care and documentation of Physical Therapy Assistants.
- Supervises Home Health Aide activities when P/T is the primary discipline involved in case
Qualifications and Requirements
- Current licensure as a Physical Therapist in the State of California.
- Graduation from a Physical Therapy curriculum approved by the American Physical Therapy Association.
- A minimum of one year’s experience as a Physical Therapist is required. Experience as a Physical Therapist in home health is preferred.
- Current CPR card required
