HumanEdge is partnered with an amazing organization to recruit strong Hospice RNs for a weekend day position. It is a 24 hour work schedule.
- Admits patients to service for Hospice
- Evaluate hospice patients as needed
- Perform assessment of patient and family, through observation, interviews, and analysis of records.
- Analyze physical, emotional psychosocial, environmental and spiritual needs.
- Begin development of a comprehensive care plan.
- Completes paperwork required by agency, government, and third-party systems
- Graduation from an accredited school of nursing; BSN preferred
- CT RN License
- A minimum of two years of nursing experience
- Home Health Care, Hospice/End of life care experience preferred
- Certified Hospice and Palliative Care Nurse (CHPN) preferred
