We're seeking a dedicated Hospital Pharmacist to join our team in Philo, Ohio, for a 16-week assignment. Responsibilities:

• Medication Management: Review and verify orders, prepare and dispense medications, including IV preparations.

• Clinical Consultation: Advise healthcare teams on medication therapy and dosage adjustments.

• Patient Monitoring: Monitor for adverse effects, drug interactions, and allergies; adjust therapy as needed.

• Patient Education: Educate patients and providers on medication use and safety.

• Collaboration: Work with healthcare professionals to develop and implement patient care plans.

• Quality Assurance: Participate in quality improvement and maintain pliance with regulations.

• Inventory Management: Manage medication inventory and ensure proper storage and disposal.

• Emergency Support: Provide pharmaceutical support during emergencies and critical events.

Requirements:

• Active Ohio Pharmacist license

• At least 2 years of recent hospital-based experience

• Proficiency with EPIC software

Schedule:

• 40 hours/week, with varied shifts including weekend and holiday rotations
