1 Hospice Case Manager RN needed!
Local: $52.50/hr, $78.75OT, $2100 gross
Travel: $20.93/hr, $31.39OT, $1533stipend, $2370gross
Start date: 6/17 or 6/24
Monday-Friday 8:00am-4:30pm
Duration: 8 weeks
Ohio license or Compact
EMR: NetSmart (experience preferred)
Job Details: Responsible for planning, coordinating and delivery quality hospice care to individuals and their families.
Manage routine care for their patients, making sure they are as pain-free and symptom-free as possible.
On Call rotation is required - will split on call with another Case Manager
Hospice experience is required
