***$1,000 signing bonus*** We are looking for an individual who is passionate about working with individuals diagnosed with mental illness, intellectual disability, or drug and alcohol addiction.

Responsibilities:

* Provide case management services to ACT clients

* Work collaboratively with clients to develop individual treatment and safety plans based on assessments of strengths and needs

* Assist clients through community-based activities and as defined by individualized service plans

* Client outreach and engagement instruction support for daily living activities

* Linking with mental health, substance abuse, and medical assessment, treatment, and rehabilitation services not directly provided by ACT

* Linking with community resources including housing, financial entitlements, employment and recreational resources

* Informal support network building

* Crisis intervention/problem resolution

* Client advocacy

Job Requirements:

• Bachelor degree Human Services, Psychology and Criminal Justice.
• Valid driver’s license and ability to drive compact company vehicle.
