Job DescriptionCompany Description
- The LVN will provide direct nursing services for adults with severe and persistent mental illness, assist with medical treatments, complete documentation for the clients.
- Provide nursing services to clients to include medication injections, assist physicians and team with caseloads.
- Educate clients on medications, and general health issues.
- Along with psychiatrist and Nurse Practitioner, nurse serves as the medical specialist on ACT team.
- Assist with medical treatments/interventions appropriately, taking initial health history on new patients and conducting a monthly or quarterly for all clients on their assigned team.
- Transport consumers to sites of medical interest (as needed) –
- This position is field-based nursing, employee will provide visits to consumers in their homes and the community.
- Other duties as assigned
