Up to 29 hours per week positions
Under general supervision of a qualified supervisor provide direct care and oversight to adults with developmental disabilities supported by a community-based program and living in their own homes or with their family.
- Reviews and follows the Service Plan developed by agencies' RN.
- Provide watchful oversight over the clients' activities.
- Participates in the development of the individual service plan for each assigned client by providing input based on observation of skills and deficits.
- Suggests goal ideas to the interdisciplinary team.
- Prepares and implements activities and plans meaningful community outings with the client that support goals and objectives identified in the service plan relating to the home.
- Uses appropriate cues, prompts, re-enforcers, and materials to promote the client's dignity and independence while adhering to the individual service plan.
- Physically assist with personal care activities such as bathing, toileting, grooming, shaving, tooth brushing, dressing, and eating.
- Assist with transfer or ambulation.
- Perform medically related activities such as taking vital signs, cleaning around tracheotomy or gtube, or administering tube feedings.
- Document tasks that were performed at each visit using the provided tracking records.
- Document any changes in conditions or vital signs.
- Implement behavioral intervention plans and document responses, as needed.
- Monitors clients to ensure their safety/security and emotional and physical needs are addressed.
- Assists with food preparation, service, and cleanup, providing supervision and assistance to clients as needed.
- Ensure home safety.
- Ensure infection control measures are taken.
- Transports client to and from community outings (relating back to the home) following all agency transportation, safety, and vehicle operation policies.
- Assures that medications are available for self-administration by the client, as refills are needed.
- Remind client to take their medication.
- Accounts for client's personal spending money.
- Maintains required records.
- Prepares documentation of client's activities that is complete, accurate, and legible and that meets organizational and accreditation requirements. •
- Maintains files of all materials in the prescribed format.
- Submit documentation weekly in compliance with policy/supervisor's preference.
- Participate in training and scheduled agency meetings.
- Other duties as assigned.
Qualifications:
- Valid Driver's License
- Dependable transportation (proof of insurance)
- CPR & 1st Aid certified (provided by agency)
- Ability to work flexible hours
Preferred Qualifications:
- High School Diploma or GED
Required Competency Skills: Must have the ability to successfully fulfill documentation requirements.
