The applicant is responsible for the provision of covered services to enrolled and unassigned patients presenting for care.
The applicant will be ensuring the patient’s care plan contains medical recommendations for clinically indicated care.
The applicant is responsible for reviewing available clinical and performance data with the team, and focusing on continuous improvement of critical team processes.
The applicant is responsible ensuring the patient has same-day access for face-to-face, telehealth, and telephone care visits during regular clinic hours
Qualification:
The applicant shall demonstrate evidence of education, training, and experience in Internal Medicine or Family Practice.
Education/ Experience :
The applicant must have an MSN from an accredited nursing program and have ANA Certification as a Nurse Practitioner in either Adult Health or Family Practice.
The applicant must have an Authorization for prescriptive authority.
The applicant must possess Three years of clinical nursing experience.
The applicant must possess a minimum of one (1) year clinical experience as a CRNP
