About Us: Evolve Managed Care Solutions is a new Behavioral Health Clinic dedicated to providing high-quality behavioral health services to the Augusta, Georgia community. We specialize in offering Therapy, Personalized Repetitive Transcranial Magnetic Stimulation (PrTMS), Care Coordination, and Medication Management to enhance the mental well-being of our patients. Job Description: We are seeking a dedicated Nurse Practitioner or Physician Assistant with a strong background in behavioral health to join our growing team. As a valued member of our clinic, you will play a crucial role in providing comprehensive care to our patients, including PrTMS, Care Coordination, and Medication Management. This position offers an exciting opportunity to work in a supportive and innovative environment, helping individuals on their path to better mental health. Ownership opportunities are available for the right candidate. Relocating to California.

Responsibilities:


- Conduct comprehensive assessments and evaluations.

- Develop individualized treatment plans for patients, including PrTMS, Care Coordination, and Medication Management.

- Monitor and adjust medication regimens to ensure optimal patient outcomes.

- Provide education and support to patients and their families.

- Collaborate with a multidisciplinary team of healthcare professionals.

- Maintain accurate and up-to-date patient records.

- Participate in continuous quality improvement initiatives.

Qualifications:


- Current Nurse Practitioner or Physician Assistant licensure in Georgia.

- Certification in Behavioral Health or Psychiatry.

- Previous experience in behavioral health, psychiatry, or a related field.

- Excellent communication and interpersonal skills.

- Strong analytical and problem-solving abilities.

- Ability to work collaboratively as part of a healthcare team.
