WeCare at Sycamore Care Center is actively seeking a motivated Social Worker for our community in Montoursville, PA.  


Key Responsibilities
·      Reviews and follows up on any concerns for the residents and staff with all incident reports.
·      Works with caregivers of the nursing unit and all departments to provide the residents with a fulfilling living experience. 
·      Keeps supervisor informed of unusual occurrences that relate to the health and safety of residents.
·      Oversees care plans, MDS, and quality improvement processes.
·      Keeps Admin fully informed of stats, progress and achievements. 
·      Must have the ability to deal tactfully with personnel, residents, family members, visitors and the general public.
·      Must be able and willing to work well with inter-disciplinary teams. 
·      Any other duties as assigned 


Skills, Knowledge and Expertise
·      Bachelors Degree in Social work or Human Services.
·      Previous experience in geriatrics or long term care.
