JOB SUMMARY:
The Music Therapist is responsible for assessing and developing a Care Plan for patients age, emotional wellbeing, physical health, communication ability, and cognitive skills through musical responses to meet the needs of patients and their families, promoting the quality of life for terminally ill patients and their families and participation of the development of the hospice interdisciplinary team care plan. The Bereavement Coordinator is responsible to plan, implement, and maintain a bereavement program to meet the needs of families/caregivers from Luminary Hospice of Indiana, LLC for up to 13 months following the death of the individual hospice patient.
JOB RESPONSIBILITIES & DUTIES:
Coordinates the assessment and delivery of grief counseling needs and services (one-to-one, groups, and ongoing follow-up) to ensure timely and appropriate services are provided to family/caregivers.
Plans for Bereavement Program development, expansion, and refinement through annual evaluation of services
Provides bereavement support to hospice personnel coping with work related grief through one-to-one counseling, supervision of interns providing one-to-one counseling, and referral to community resources.
Provides bereavement information and referral services to callers from the community. Expands and maintains community resource list as needed.

JOB REQUIREMENTS & QUALIFICATIONS:
Bachelor's degree in therapy or related fields, required. Master's degree in Music Therapy or related healthcare field, preferred. Maintain an active Board Certification in Music Therapy for the state practicing. Maintains an active CPR certification. At least 2-5 years of experience in the healthcare management, hospice or palliative care and/or training in end-of-life care, preferred. At least 1 year of experience with Medicare Hospice Certification as well as CHAP guidelines, preferred. Knowledge hospice care and the services provided to patient and family/caregiver through an interdisciplinary group.
Proficient in MS Office applications and ability to learn department and job-specific software systems
Demonstrate organizational skills. Demonstrate effective verbal and written communication skills
Demonstrate analytical skills when problem-solving. Demonstrate high attention to detail and a high degree of accuracy.
