Seasoned Recruitment is currently seeking a committed and experienced Primary Care Nurse Practitioner to join our interdisciplinary team (IDT) and deliver direct medical care to PACE (Program of All-Inclusive Care for the Elderly) participants. In the role of a Primary Care Nurse Practitioner, your responsibility will be to collaborate closely with the IDT to formulate and execute care plans designed to enhance the health outcomes of our participants.

Responsibilities:

• Must perform in-person comprehensive assessments including initial, semi-annual and unscheduled assessments and continuous medical coverage to participants
• Must be able to work with physicians and Interdisciplinary Team (IDT) to develop care plans for each participant
• Must be able to work with physicians and Interdisciplinary Team (IDT) in discharge planning from acute or long-term settings
• Must help maintain care of participants in the nursing home, making regular visits as dictated by participant need
• Must work collaboratively with physicians, community-based PACE physicians, and IDT to oversee and ensure effective care coordination
• With the direction of the Medical Director, review outcomes and utilization data, including hospitalizations, and use of specialists to establish best practices and policy improvements
• Participate in rotating night and weekend call shifts.

• Graduation from an Accredited School of Nursing
• Certificate as a Nurse Practitioner with Prescriptive Authority
• RN State License
• Minimum of two years of experience working with the senior population
• Current knowledge of chronic care/geriatric issues and best practices
• Reliable transportation
