The Adult Day Care Center in the Upper West side of Manhattan is seeking is seeking a full time Social Worker to join our team! The work hours is Monday to Friday , with these work hours:

• Monday and Thursday= 8:30 am to 4:30 pm
• Tuesday, Wednesday and Friday = 9:30 am to 5:30 pm

Job Responsibilities:

• Work as part of an interdisciplinary team to provide comprehensive MDS and psychosocial assessments
• Coordinate and facilitate care planning and completion of all documentation
• Facilitate team and family meetings, education on older adults rights and well being needs of the vulnerable elderly population..This involve extensive work with family members and friends as well.

Job Qualifications:

• MSW from an accredited School of Social Work.
• Valid New York State License in Social Work.
• Experience and/or expressed interest in working with adult and geriatric population.

• Experience in adult day care center preferable .

• Solid written, communication and organizational skills
