Job Title: Family Nurse Practitioner (FNP)

Location: Phoenix, Arizona

Pay Rate: $65 - $85/hr on W2, Based on experience

Duties and Responsibilities:

• Deliver primary care services with a client-centered, harm-reduction focus.

• Administer medications such as psychiatric, Vivitrol, Suboxone, or Subutex.

• Refer patients to specialists, hospitals, community agencies, or in-house staff as needed.

• Maintain accurate and timely records, including medical evaluations and physical examinations.

• Provide emergency care and treatment for conditions such as anaphylaxis and acute asthma.

• Work flexible schedules in a hybrid model at multiple locations.

• Advocate for clients' needs to ensure access to effective care and resources.

• Participate in on-call coverage to support clients.

Essential Requirements:

• Must be Licensed in Arizona.

• Active ACHHHS ID

• Current DEA License

• At least 2-3 years of Primary care experience

• Previous experience with medication-assisted treatment and family medicine is highly preferred

• CPR certified within 3 months of hire

• Ability to pass a credit check, background check, and drug screening.
