Responsibilities:

• Perform initial and ongoing PT evaluations that evaluate mobility, strength, and current level of function according to APTA standards of practice.
• Develop and implement an individualized plan of care for each patient based upon evaluations, goals, and attainable outcomes.
• Coordinate care with an interdisciplinary team to restore patients to their best level of function, increase independence, and improve quality of life.
• Reinforce patient accountability to ensure consistency and compliance with their treatment program.
• Maintain medical documentation and policies/procedures; communicate to patients and caregivers.
• Create a post-treatment plan to continue their improved quality of life; recommend assistive equipment as needed.
• Maintain professional and technical knowledge in orthopedics, sports, pelvic floor, pain management and/or vestibular rehabilitation by attending educational workshops, reviewing professional publications, and participate in monthly continued education staff presentations.

Qualifications:

• Valid Colorado physical therapy license.
• Prefer 2+ years’ experience in orthopedics, sports, pelvic floor, pain management /or vestibular.
• Degree in Physical Therapy from an accredited PT program .
• High level of empathy, a positive outlook, and the ability to motivate patients to push their limits in a safe and effective way.
• Understands responsible use of facility equipment, best practices for use, and preventative maintenance procedures.
