• This day shift position is full-time, 80 hours per pay period, Monday – Friday, and an occasional Saturday, Benefited.
• This position will be a hybrid/mix of outpatient and inpatient rehabilitation.

Essential Duties:

• Conducts in-depth physical therapy evaluations, assessing patients' physical conditions and functional limitations.
• Interprets evaluation findings, synthesizing information to form a comprehensive understanding of patients' needs.
• Develops a prognosis and personalized plan of care based on evaluation results and evidence-based practice.
• Maintains accurate and detailed documentation following accepted standards, ensuring comprehensive records of evaluations, progress notes, and treatment plans.

Educational Qualifications:

• Minimum of Bachelor of Science (B.S.) degree in Physical Therapy from an accredited program
• Doctorate in Physical Therapy from an accredited program preferred
• Current Wisconsin license as a Physical Therapist
