Are you passionate about making a real difference in your community? LHH is looking for a dedicated Bilingual Case Manager to join an amazing non-profit in Oakland, CA. This is your chance to be on the front lines, providing essential services to those with complex needs—right where they need it most.
What You’ll Do:
Build meaningful relationships through outreach at various community sites.
Develop personalized care plans and coordinate services that change lives.
Help patients secure and retain housing, providing the guidance they need.
Offer health education on harm reduction and risk mitigation.
Be a calm, guiding hand during critical moments.
Keep track of your impact with accurate, timely documentation.
What You’ll Bring:
High School diploma or GED (Bachelor’s in Social Work is a plus!).
3+ years in community-based health or social work. Bilingual in English/Spanish is a must.
Strong organizational, problem-solving, and interpersonal skills. Microsoft Office proficiency required.
