Title: Social Worker LMSW
Job Type: Permanent / Direct Hire
Setting: Hospice/Home Health
Location: Great Bend, KS
Schedule: Full-time days Mon thru Fri, normal office hours, with occasional evenings

The Social Worker provides psychosocial services to patients, families, and caregivers:
-Provide group counseling, group education, family conferences and individual counseling and support sessions
-Assess patients' psychosocial distress and helps facilitate patients' knowledge about psychosocial, emotional and financial implications facing home health and hospice patients
-Complete psychosocial reassessment as needed, management of patients' psychosocial needs; community coordination, treatment team planning intervention, utilizing knowledge of community and government resources for the patients and their family

REQUIREMENTS:
-Master's degree in social work from an accredited School of Social Work required
-Current social worker licensure LMSW or LSCSW with the Kansas State Board of Social Work
-Knowledge of principles, methods and techniques in performing general social work services and adapting or modifying standards of practices for care of specific medical cases
-BLS upon hire or obtained with 2 weeks of hire
-Open to considering new grads; will provide supervision and training if needed!
