Role and Responsibilities
Provide one or more of the following 245d services below:
HOMEMAKING:
Services that help a person manage general cleaning and household activities.
- Washing dishes
- Sweeping, mopping, vacuuming
- Light dusting
- Changing linens
- Taking out trash
- Laundry
NIGHT SUPERVISION:
Overnight assistance and supervision provided by an awake staff in a person’s own home. A person’s own home means a setting the person owns, rents or leases that is not operated, owned or leased by a provider of services or supports.
- Carrying out the person’s positive support programming and transition plans
- Reinforcing skill development supports (e.g., individualized home supports)
- Assisting with activities of daily living (ADLs)
- Assisting with instrumental activities of daily living (IADLs).
ADULT COMPANION CARE SERVICES:
Non-medical care, supervision and socialization provided to a person age 18 or older. Adult companion services cover services that help a person work toward a therapeutic or community integration goal in their support plan.
- Attend a movie with the person to practice coping skills to manage their social anxiety
- Go with the person to a community event to reduce their social isolation
- Play a board game with the person to enhance their fine motor skills
- Provide verbal instructions or cues to the person to help them complete a task
- Assist or supervise the person with tasks such as laundry, light housekeeping, meal preparation and shopping
PERSONAL SUPPORT:
Services provided in the person’s home or community to:
- Achieve his/her full potential
- Increase his/her independence
Meet community inclusion goals that are important to and important for the person and based on an assessed need.
INDIVIDUALIZED COMMUNITY LIVING SUPPORTS:
- Active cognitive support
- Adaptive support service
- Activities of daily living (ADLs)
- Household management
- Health, Safety and Wellness
- Community engagement
24-HOUR EMERGENCY ASSISTANCE:
On-call counseling and problem solving and/or immediate response for assistance at a person's home due to a health or personal emergency.
RESPITE CARE:
Short-term care services provided to a person when the person’s primary caregiver is absent or needs relief.
Qualifications and Education Requirements
Required:
- Experience with 245d requirements and personal support or PCA work.
- Ability to remain awake for long hours of night if providing night supervision.
- Experience working and communicating with people with disabilities.
- Ability to manage conflict and de-escalate.
- Excellent communication skills.
Preferred:
- GED, High School Diploma or higher education.
- First Aid Certification
- 245d Trained
- Experience using electronic time sheets and EVV technology.
- Experience working in a diverse environment with people of different backgrounds.
- Ability to communicate in other languages (Somali, Spanish, Arabic, Karen, etc.)
