1 year experience as LPN in LTC / SNF or like setting required, 2 years preferred.

Previous LPN travel experience preferred.

Job Duties:

Nurse will conduct thorough and ongoing assessments of patients, including monitoring vital signs, assessing pain levels, and identifying changes in condition.

RN / LPN will develop and implement individualized care plans based on patients' medical history, current condition, and physician orders, ensuring holistic and patient-centered care.

RN / LPN will safely administer medications, including intravenous (IV) therapies, and monitor patients for any adverse reactions or side effects.

Registered / Licensed Practical Nurse will continuously monitor patients' status, recognizing and responding to signs of deterioration, and initiating appropriate interventions.

Provide comprehensive wound care, including dressing changes, wound assessments, and implementation of appropriate wound management strategies.

Educate patients and their families about disease processes, treatment options, post-operative care, and lifestyle modifications to promote recovery and prevent complications.

Maintain accurate and detailed documentation of patient assessments, interventions, medications, and outcomes in electronic health records (EHR) systems.

Assist in the development and implementation of discharge plans, including arranging follow-up care, coordinating home health services, and providing discharge instructions.
