Unlimited Care, Inc., a leading licensed home care service agency with 14 branch offices in NY, has an office position for an experienced, caring and compassionate LPN.

The licensed practical nurse primarily evaluates the paraprofessionals skills competency, conducts annual paraprofessional performance evaluations and on occasion provides paraprofessional supervision visits in the field. The LPN also functions as an assistant to the registered professional nurse with paraprofessional in-service education, tracking physician orders and reviewing paraprofessional progress notes. The LPN reports directly to the Supervisory Nurse in the branch.

Responsibilities:
• Evaluates initial and on-going paraprofessional skills competency and completes annual performance evaluations.
• Tracks Physician Orders.
• Assists with paraprofessional in-service education.
• Reviews paraprofessional documentation and provides re-education.
• Assists the Supervisory Nurse with other compliance requirements.

Job Requirements:

• Currently licensed as a practical nurse in NY state
• 1 year of experience
