Kenosha Community Health Center and Pillar Health is seeking an experienced Program Director of Integrated and Value Based Care. This position is grant funded.
POSITION SUMMARY
The Program Director of Integrated and Value Based Care is a leadership role designed to utilize organizational and systems thinking to develop a care management system capable of implementing Evidenced Based Care (EBC).
Additionally, this role will analyze patient/population health data and develop effective strategies to achieve practice, quality and cost-effectiveness outcomes and advise the needs of the KCHC/Pillar Health Value Based Purchasing (VBP) of the future.
The Program Director will work collaboratively with the clinical and operational leadership including Chief Clinical Office (CCO), site Medical Director, Director of Clinical Practice Transformation, and Clinic Operational Manager (COM) to assure the Pillar Health Care model of integrated care is aligned to achieve financial and care access outcomes.
The initial focus of this role will be to support project implementation for Advancing Health Equity and Improving Whole Person Care Outcomes for Vulnerable Patients Using an Integrated Medical-Dental-Behavioral Health Care Model.
ESSENTIAL FUNCTIONS
· Using analytics to evaluate populations and provider panels, advise best practice to develop a care management system and advise on priorities to participate in Value Based Purchasing (VBP) needs of the future .
· Evaluate existing resources and workflows developing a system to implement best practices required to achieve organizational outcomes related to the integrated model of care.
· Evaluate and advise best practices for team-based care with an emphasis on team building.
· Evaluate and implement roles deployed to provider care teams to maximize effectiveness in day-to-day functions.
· Evaluate and implement care management nursing workflows working independently and with providers. optimize provider panels by interacting with hi-risk patients to improve outcomes.
· Evaluate all nursing roles and best processes to fully implement the Patient Centered Medical Home (PCMH)program
POSTION REQUIREMENTS
Education/Experience
· Doctor of Nursing Practice (DNP) degree preferred. Masters of Science in Nursing (MSN) minimum requirement.
· Minimum of 3 years operational leadership required.
· Proven experience in systems design and population health management required.
