Pinnacle Physicians Group, the largest independent primary care group in Northeast Philadelphia is seeking a Nurse Practitioner/ Physician Assistant to become a part of our team! This full-time office based position is located in the far Northeast convenient to Interstate 95 and Route 1.

You will focus on providing high quality patient care as part of a healthcare team.

Responsibilities:

• Diagnose and treat acute, episodic or chronic illnesses
• Prescribe all necessary medications and treatments
• Perform comprehensive physical examinations of patients
• Develop and implement patient management policies and procedures
• Facilitate referrals to other healthcare professionals and medical facilities
• Communicate with collaborating physician or specialist regarding patient care
• Help facilitate and implement population health, preventative health guidelines and care to our patients

Qualifications:

• Previous experience in primary care is required
• Ability to build rapport with patients
• Ability to thrive in a fast-paced environment
• Experience with or familiarity with electronic medical records(EMR's)
• Strong leadership qualities
• Experience with Value Based Medical Care/Population Health is a plus
