You are a PTA who:

• Holds a current License/certification in Wisconsin
• Values building connections with their patients.
• Likes working independently while backed by a collaborative team.
• Has great written and oral communication
  

Job Duties Include:

• Provide Physical Therapy treatments to patients according to treatment plans developed by the Physical Therapist
• Collaborate with the Interdisciplinary Team to identify and assist with obtaining patient specific DME including mobility aides, wheelchairs, cushions, orthotics, and adaptive equipment.
• Maintain daily records of treatments and document progress notes on patients as directed by the Physical Therapist
• Collaborate with Physical Therapist and clinical team to develop and educate patient and/or care givers on safe discharge plan.
